1.A) LABORATORIO CLÍNICO AMARE S.A. DE C.V., with fiscal address in vasco de gama, 295 fraccionamiento costa azul, cp. 39850, Acapulco de Juarez, Gro. Mexico is responsible for collecting your personal data, the use that is given to them, and their protection.
B) Your personal information will be used to provide the clinical analysis services you have requested, since in case of not having this information we would not be able to inform you about the status and result of the same, as well as evaluate the quality of the service we provide. For the aforementioned purposes, we need to obtain the following personal data: your name; domicile; Date of birth; country and entity of birth; nationality; occupation, profession, activity or line of business to which it is engaged; landline or cell phone numbers; emails, Unique Population Registry Code (CURP); Federal Taxpayers Registry Key; as well as, the serial number of your Advanced Electronic Signature. We inform you that, in order to comply with the purposes set forth in this notice, sensitive personal data will be collected and processed, considering as such those that refer to the state of your personal health.
C) We undertake that they will be treated under security measures, always guaranteeing their confidentiality unless requested by the competent authority *, according to NOM-007-SSA1-2011 in section 4.4.3 and 5.1.1 and article 10 paragraph VI) of the Federal Law on Protection of Personal Data held by individuals, or in case your results are critical, your treating physician will be informed. CH. 01), Taeniasis (Coproparasitoscopic), Streptococcal Angina (Hemolytic S. B Culture), Brucellosis (Rose Bengal, 2-Mercapto-Ethanol, SAT), Tuberculosis (Positive Bacilloscopy, Mycobacterium Culture), Syphilis (VDRL Positive, FTA-ABS Positive), Viral Hepatitis C (Hepatitis C Positive ELISA, Enzyme Immunoassay and Supplementary Test (RIBA HCV 3.0), (LIATEK HCV III), Viral Hepatitis «B» (Anti Hepatitis B Positive Elisa, Ag POSITIVE), VIRAL HEPATITIS «A» (Elisa Anti Hepatitis A Positive, Ag HAs positive), hiv/aids serotypes (elisa positive, Western Blot Positive, Viral culture), Gonococcia (Gonococcal Smear, Gonococcal Culture), Human Papillomavirus (Coilocytes in cervical cytology), Meningococcal Meningitis (Smear with Meningococcus, Meningococcal Culture), Malaria (Thick gout) and Dengue (IgM positive to dengue virus) your personal data (Name, Age, Address, Telephone) will be notified to the Department of Epidemiology of the State of Guerrero.
D) You have the right to access, rectify and cancel your personal data, as well as to oppose the processing thereof or revoke the consent you have given us for this purpose, through the procedures we have implemented. F) Likewise, we inform you that your personal data may be transferred and processed within the country, by persons other than this company, in the case of very specific and unusual studies, (information that will be made known by the receptionist and/or phlebotomist who takes your data and service order).
G) You have the right to access your personal data that we have and the details of the treatment thereof, as well as to rectify them if they are inaccurate or instruct us to cancel them when you consider that they are excessive or unnecessary for the purposes that justified your request for a clinical analysis order. To do this, it is necessary that you submit your request directly to our personal data department either by telephone or electronically, where the personal or sensitive data on which you wish to exercise your ARCO rights (access, rectification, cancellation, and opposition) will be taken. We will have a maximum period of [5 days] to attend to your request and we will inform you about the origin of it either by telephone or electronically.
